Accepted Insurance Plans
Because statements and billing costs have become so expensive and, in an effort to keep your medical costs down, please be prepared to pay for office visits at the time of service. Your secondary insurance will be billed for surgeries only; therefore, responsibility for payment is your obligation. Payments may be made by cash, check, MasterCard, Visa, or Discover. A charge of $10.00 will be made when personal checks are returned for insufficient funds.
We will be happy to discuss fees with you and provide an estimate of proposed fees for any procedure in advance. Since the insurance industry is changing so rapidly, you should check your insurance directory to see if we are listed as participating physicians.
